山形瑞秋（Rachael Yamagata，1977年9月23日－）是名美國創作歌手。山形瑞秋的父親是第三代日裔美國人，母親則有意大利與德國血統。她的作品有《BeBeYourLove》、《WornMeDown》等。
山形瑞秋的故事從一支名叫“Bumpus”的芝加哥樂團開始，2001年瑞秋與樂隊吉他手男友分手後單飛來到紐約發展個人音樂事業。2003年首張個人EP的推出預示着瑞秋個人音樂生涯的正式開啓，2004年備受讚譽的首張個人專輯《Happenstance》推出後，一個在愛情裏斷腸心碎的苦情歌女民謠詩人形象被人們所熟知。
在瑞秋整個音樂生涯裏，人們對她的印象似乎已經被她曾發行過的三張專輯和五張EP定格。《Dealbreaker》《You Won't Let Me》《Love Hurts》等令人心碎的歌曲形成了人們對瑞秋的既定印象。她曾說“我總是通過寫歌來理清我的感情和我經歷、眼見的一切，寫歌已經成了我將痛苦轉化釋懷的途徑。沉浸在心碎中對我來說是一件有趣的事情”。
